Congestive Heart Failure Education Program
What is it?
As part of her nursing school project, one of the hospital's staff RNs developed a congestive heart failure education program for patients. This free eight-week program is implemented through the hospital's Cardiac Rehabilitation Department. Each session includes an education program and an assessment by an RN.
Who is it for?
All members of the community who wish to improve their quality of life and effectively manage congestive health failure.
Why do they do it?
Congestive heart failure was the hospital's number-two discharge diagnosis in the prior fiscal year, so this program is expected to have a positive impact on the medical management of this chronic disease.
