Community Med Card Initiative

What is it?
Members of the Utah Hospitals and Health Systems Association, in cooperation with the Utah Adverse Drug Events Work Group, recently launched the Community Med Card Initiative. The program offers providers and consumers a downloadable resource for recording medication information. The Med Card is available in a full-size page as well as a wallet format for the convenience of consumers. The med card is available online at www.uha-utah.org.

Who is it for?
Providers and consumers of health care.

Why do they do it?
The effort is part of the Association's work to improve medication reconciliation efforts within the hospital setting, as well as for individual use by family members caring for the elderly. Using the Med Card helps prevent medication errors by allowing health care providers to check that patients are receiving the right dose, as well as check for drug interactions and allergies.
